Question : 1) While sending the addendum do we send All the 5 pages OR only the first signed page ? 2) On Format B there are 2 columns 1 under the heading UNIT and the other Price per unit. Both are asking for per hour ceiling. So how do you wish for us to reply to the Rate request ? 3) Can the Rate be in a Range e.g $50 - $65 per hour ? 4) Is it ok if we provide some info about our company and our references ?  
Answer : 1) Only the first signed page is required. 2) Unit is term HOURLY RATE , EACH, BOX, etc, Price per unit is the $ amount you are submitting. 3) No, the price must be a single dollar (ceiling amount, NOT a range. 4) You may provide additioanl information but the bid will only be evaluated on the items required.

First page of RFP with form at bottom completed 2. Vendor Information Form 3. Bid Proposal Form - Format B You do not require resumes.  
Answer : Return the following documents: The bidder qualification form. The Vendor Information Sheet The Bid Proposal Form (Format B) Occupational License, if applicable. Listing of employees and certifications for the areas you are submitting for. Resumes will be required from awarded vendors when jobs are quoted. All of this information was in the bid and has additionally been listed in the addendum.
