(10)

Bid Number : 088-EE03  Opening Date : 06/20/2006
Bid Title : A-LA-CARTE PROGRAM
Buyer : R. Johnson
Subject : Reference to other bids for comparison purposes
Question : I am inquiring about bid 088-EE03 the A-LA-CARTE PROGRAM. Is there a website where I can check competitor’s bids? Please let me know.
Answer : M-DCPS bidding process is a sealed bid process. Procurement does not know who is bidding or what price is bidded on any bid until the opening date after 2:00PM (as is the case on this bid and most bids in general...however check each bid to make sure of the opening date and time.) At the time of the bid opening, the bid box is unlocked and the bids are logged in and read upon request. The bid opening ceremony is open to the public to attend. This bid replaces several individual item bids. Please check the procurement website at: http://procurement.dadeschools.net where you can click on GENERAL INFORMATION, then BID SOLICITATION, then SEARCH BIDS BY BID NUMBER OR TITLE. For example, one can type in the word "FRESH" and find several bids that were previously awarded and are now part of this bid. The award information states who won the bid, which items were awarded, and what was the awarded price. A separate tab sheet is available for most bids that states all bids received and all items and prices bidded on by each company submitting a bid. Should you not find any bid covering the items you want to bid on this bid, please send an e-mail or fax to the buyer stating what item you are requesting previous award information on. The buyer can then find the related documentation and inform you on the procedure to obtain a copy of the previous award item. Please remember that the information above is the historical cost of past awards. M-DCPS Procurement Management Services hopes to achieve through this competitive bidding process the awarding of items at the same or better pricing that our previous bids, where applicable.

Subject : Supplemental Information linking restaurants to schools in bid
Question : In looking over the bid format, I do not see any attachment similar to attachment A in previous bid packages which would link schools with particular restaurants which would be supplying product to each school. We have multiple locations in Dade County which will be participating. Am I missing something?
Answer : Please Answer "Y" on Format B SUPPLEMENTAL FORM to indicate which High Schools/Vocational Centers your company intends to service. Upon award, those vendors awarded will be requested to fill out a separate attachment (Attachment DC-Direct Contact) which will provide M-DCPS the information needed to link which location(s) shall service each school listed in the bid.

Bid Title : A-LA-CARTE PROGRAM
Buyer : R. Johnson
Subject : Supplemental Documents mentioned on bid.
Question : 1) On the Special Conditions Section, Item # 9. It says that successful bidders are required to have insurance coverage, as specified in the indemnity and insurance forms, attached hereto and made a part of this bid. My questions is, Where can I find this insurance forms, because I cannot locate them on the bid? 2) On the Special Conditions Section, Item # 21. It says each and every distribution agent is/are required to complete and submit with their bid, the U.S. Department of Agriculture Certification Form contained herein. My questions is: Where can I find this certification form, because I cannot locate it on the bid?
Answer : With respect to insurance coverage, the documentation will be requested upon awarding of the bid to successful vendors. With respect to the USDA Certification Form, this applies only if any of the items in the bid qualify according to USDA standards for the cost to be reimbursed back to M-DCPS from the USDA. At the time of bid release, none of the items are intended to be used by M-DCPS for reimbursement purposes. However, this statement is provided in the case that the purchase of additional items in accordance within the special conditions of the bid would qualify for the reimbursement. At that time M-DCPS would determine the awarded bidder to be a distribution agent. When M-DCPS decides to approve an item to submit for reimbursement, at that time the distribution agent would be required to submit the documentation needed.

Subject : Pizza items on bid- filling out the Format B Excel Worksheet
Question : According to Addendum 1 on the Bid Proposal Form, Items 32 to 35 (Pizza) now says on the item description 14-16 inch pizza pies. My questions are: 1) Are the pizza to be any size between 14 and 16 inches or only 14 inches or 16 inches? 2) How do we specify or note which size pizza we are bidding for? Thank You very much for your time and help.
Answer : Within the items on the Excel Spreadsheet (FORMAT B) in the Pizza Catgory (Items 32-35), the bidder is to state what size pizza pie they shall offer. The size of the pizza needs to be stated and the size needs to remain consistent throughout the entire bid period. It is the bidders discretion to decide what size to bid on. It is recommended to follow standard industry measurement units (i.e. whole inches) when determining what pizza size to offer. The bid states 14-16 inches, therefore 14, 15, or 16 inches would be considerable acceptable. The bidder may enter their size of pizza in the boxes beside the BRAND label listed on the Format B Excel Spreadsheet (i.e. 1ST LINE: World Cup Pizza and on the 2nd LINE: 14(or 15 or 16)inches. Remember also that bidders may be requested to submit samples as part of the bid evaluation process. In addition, each high school and/or vocational school has the choice to select awarded vendors on this bid, so keep this in mind with respect to pizza size and price.

Bid Title : A-LA-CARTE PROGRAM
Buyer : R. Johnson
Subject : Pizza Category- Number of Slices
Question : I saw that now you may choose your size of pizza (14 inches to 16 inches) Do all of them needs to be cut into 8 slices or could they be cut into more or less slices?
Answer : The Department of Food and Nutrition has been consistently serving pizza pies to high schools and vocational centers in the format of 8 slices per pizza pie. This is why the number 8 is filled in on the Format B Excel Spreadsheet. With the number 8 filled out in the slices per pie field, the bidder can see the price per slice calculated out for the their reference. HOWEVER, REMEMBER THE EVALUATION UNIT FOR THE PIZZA CATEGORY IS EACH PIE. Therefore, please make sure your bid offer is the price for each pizza pie.

Bid Number : 088-EE03  Opening Date : 06/20/2006
Bid Title : A-LA-CARTE PROGRAM
Buyer : R. Johnson
Subject : Items on Bid- Beverages Category
Question : My company has several questions regarding the Beverages category on the A-LA-CARTE Bid: 1) Quantity Verification: Using the quantities and case packs listed on the bid, we would like to clarify the total finished beverages and cups. Usually the cup usage closely matches the finished beverages, so we thought we may have either incorrectly interpreted the information or a typo may appear on the bid. 2) Location information: How much equipment is involved in this bid? (This may be listed in the PDF sheet I was not able to access, if so I apologize) (a) Quantity of coffee brewers (b) Quantity of cappuccino/hot chocolate machines (c) Quantity of coffee cooler machines 3) Brands accepted: (a) Are we allowed to offer more than one brand for you to consider? We have several branded products (coffee) that you may be interested in. (b) Item 21 Coffee Cooler - would you accept the same coffee cooler product packaged in a ready to go 11 oz shelf stable plastic bottle? We have other bids awarded from counties in Florida that have selected this as an option. 4) Let us know your thoughts.
Answer : Thank you for your questions. To answer them in order: 1) The quantities listed on the Excel Worksheet Format B (items on bid) are estimated quantities. M-DCPS is under no obligation to purchase the quantities listed on the bid. However, the estimated quantities are based on actual usage patterns when available. 2) Serving equipment: The Department of Food and Nutrition expects that there would be at least 2 per high school of each machine. 3) (a)More than one brand can be bid, but would require separate bid submissions. Brands may be tested as part of the evaluation process according to procedures listed at http://nutrition.dadeschools.net and if liked by students, there are options for purchasing those additional items after award according to the special conditions listed in the bid documents. 3(b) Please bid what is specified on the Excel Format B document. We may consider other sizes at a later date under purchase of additional items in special conditions. Remember that in this category all items must be bidded on in order to be considered for award in this category. 4) Thank you for your questions.
