Subject : BID QUESTIONS AND ANSWERS: 
Question : 1. Do I have to sign and send with my Bid, the original signed copy of the Addendum No. 1 form? 2. Do I need to send with my bid, the original Bid Proposal (Format B) form, completed? 3. Do I have to complete and send the Vendor Information Sheet with my bid? 4. Do I need to list 3 references and license information on the sheet provided in the bid package for that purpose? 5. Should I attach a copy of my occupational license and other licensure documents with the bid? 6. How many visits per month need to be rendered per location? 7. Will the number of visits vary from location to location? 8. How many facilities in total are in need of maintenance and/or repairs? 9. The addresses of said facilities? 10. If known what would be the highest horsepower of the submersible pumps within all of the facilities?  
Answer : 1. Yes, in order for us to consider your bid. 2. Yes. 3. Yes, we would like that. 4. Yes. 5. Yes. 6. This is not a scheduled maintenance service type bid; therefore the number of visits to any site, per month, is unknown. The Primary Vendor will be notified of emergency repair services required, when know and deemed an emergency, and all others repairs or replacements work will be solicited via a Request for Proposal, from all contract awardees. Planned repair projects will be repairs that are needed, but are not emergencies, or necessarily urgent, such as a control panel or pump upgrades. 7. Yes. 8. Unknown at this time, however, the Addendum No. 1 package list MDCPS – sewage lift stations and site locations, which may require repair or replacement service during the contract period, but no guarantees. Additional sites may be added, as new stations are brought on line. 9. Please refer to the MDCPS-SEWAGE LIFT STATIONS list, in Addendum No. 1 package, for the location of existing systems. 10. The largest pumps that we currently have are 47 horsepower.
