The only four (4) documents you must return are as follows:
1) Bidder Qualification Form (must be signed; preferably with blue ink)
2) The Bid Proposal Form (Format B) Its the page you must enter only one (1) percentage discount, even if you represent several different manufacturers products)
3) Vendor Information Sheet (must complete and send with your bid)
4) Occupational License (See Special Conditions 25.)

· Catalogs or Price Sheets are not needed at this time. If required, we will contact you.
· After bids are opened and evaluated, the District will recommend to the Board, a list of responsible and responsive bidders, who can provide the types of products listed in Special Conditions 2., and provide a discount greater than 0%.
· Once the Board grant awards to vendors on the recommendation, those vendors may begin receiving from us, Request for Quotations (RFQ), for the products we anticipated needing.
· (RFQs) will contain a list of the items, specifications, estimated quantities, and a term pricing should be held for (3 mos., 6 mos., or one (1) year).
· When Procurement Management evaluate all responses to the (RFQ), an award will be made to one and or multiple vendors; depending on the structure of the (RFQ).
· A PRICE AGREEMENT between M-DCPS and the vendor is established at this point.
· Please note, the price(s) you submit on the (RFQ), should be at least the (mfg. list price less the discount percentage you entered on the Bid Proposal Form, (Format B), of the Bid you will submit on February 26, 2004, at 2:00 p.m. e.g. Mfg. List ($150) less 5% Discount ($7.50) = $142.50 M-DCPS price). However, vendors may wish to submit, aggressive, competitive pricing, because M-DCPS will enter into a Price Agreement with the vendor(s) offering to lowest Net Price on the (RFQ).
