Question : Q1.-Page #3, part B, paragraph #3 indicates that “agency’s licensure” is required. Further, in the same paragraph it is indicated that “a copy of each clinician’s license” needs to be submitted. Can you please clarify this? If al of our clinician’s are Master’s level and are under supervision of licensed supervisor, but not everybody is licensed individually, can our agency apply for this RFP?

Q2.- Page #4, paragraph #6 is asking for two letters of reference. Are two letters needed for each counselor or are two letters need for an agency overall?

Q3.- Page #4, paragraph #12 is asking for security clearance. Is security clearance needed prior to submission of this RFP, or is it needed prior to contract execution? If security clearance is needed prior to submission of RFP can we attach DCF clearance, since there is no time left to go through School Board procedure.

Q4.- Should insurance certificate be attached with RFP package?

Q5.- Page #10 indicates that Affirmative Action Employment Breakdown form needs to be completed. Should this form be completed and attached with RFP, or should it only be completed later, if we are awarded a contract?

Answer : Answer 1: The agency’s licensure is required and clinicians have to be licensed.

Answer 2: Two letters of reference is needed for each counselor written within one year of this RFP.

Answer 3: If awarded the security clearance will be required.

Answer4: Yes

Answer 5: Should be submitted with the RFP
