Subject : Filling out items on the bid. 
Question : I would like to know if all of Format B needs to be filled out when it is a duplication of the main bid package? Is there a difference in the items and if not which one do we complete? 
Answer : The Format B (Items on Bid 007-GG03)is identical in both Adobe and Excel formats. It is stated in Special Conditions #20 the following: BID SUBMISSIONS: In order to facilitate the evaluation, vendor(s) is/are requested to submit in electronic format the Excel file, Format B (Bid Proposal Form) on a CD-ROM, 3½” Floppy Disk, or Jump Drive with the COMPLETE Bid Package. In addition, one printed copy of the COMPLETE Bid Package shall be submitted as the bid, for evaluation. In the event of a discrepancy in manufacturer code(s) and/or packaging on the electronic format, the bidder(s) shall so indicate on the blank text boxes on the electronic format. Should the bidder(s) not be able to enter the discrepancy on the electronic format, the bidder shall so indicate the discrepancy on the printed copy. THE PRINTED COPY OF THE BID SHALL BE USED AS THE DOCUMENT FOR THE EVALUATION PROCESS.

Subject : Samples needed with bid submission. 
Question : If item states to send samples and we bid as specified, do we need to send samples anyway? 
Answer : Special Conditions #11 in the bid documents state the following: Bidder(s) listing an ALTERNATE product and/or brand(s) may be required to submit samples, to be evaluated according to the standards of Materials Testing at the Department of Food and Nutrition. Procurement Management Services can ask for samples to be sent for evaluation. The samples amount listed in the Format B (items on the bid) Excel Spreadsheet are the quantities a bidder is to send upon notification from Procurmenent Management Services. Failure to delivered samples as required may be reason(s) for rejction of bid(s).

Question : Can you tell us what is the length of the gloves are on item 20 and 21? 
Answer : Bidder(s) are to bid items that meet or exceed the specifications on the Format B (items on bid) Excel Spreadsheet. If one is bidding an alternate product and/or brand(s) other than the approved brands and model numbers listed on the Format B, one may be required to submit samples, to be evaluated according to the standards of Materials Testing at the Department of Food and Nutrition. Please see the previous Q and A regarding submission of samples.
